All info on the EBB website and very self explanatory.
List of contractors and reviews on the website too.

My contractor did the application took before and after photos and videos and set up the final inspection with the city inspector.
I just paid the contractor the delta above the CA reimbursement of $3000.

Registration is closed right now but it is opened periodically.

Let me quickly clarify the #s
Total cost $3700. Out of which the distribution was the above as quoted.
